Nikki (human) and Maple (dog) would never have met, and bonded so deeply, without the care and support of the team at Dog Rescue Newcastle and all involved. We each have our own profound story of rescue, re-homing and re-connecting to something far deeper ... so we are paying it forward with proceeds from every order you make, making a difference to an animal in distress.

• The below questionnaire has been designed to help determine the most appropriate formulas or protocol for your beloved animal. This is an essential requirement for us to assist in providing optimum support and dispensation of our unique plant medicine formulas.
• Ensure you have at least 15 minutes to complete the form in one sitting, or have essential information handy elsewhere on your computer for you to copy and paste as needed as you will not be able to save the form and come back to it.
• As you are completing this form on behalf of an animal, please answer questions as comprehensively as possible for assessment purposes.
• Initial investment - $80 – payable upon submission of form. This includes a Consultation with one of our Qualified Animal Care team to ensure you are recommended the right formulas and given the right information to assist the health of your beloved animal.
• Once your form is submitted you will receive a welcome email that contains important information about your consultation and our Frequently Asked Questions (FAQ's)
